We will ask about your functional recovery, the economic impact to you and your family, and pain and pain medication usage. All questions are solely for the purpose of understanding your recovery experience.
Understanding your recovery journey after you leave the hospital will help guide us in the future care of patients like you.
A team member will reach out at key points in your recovery to complete a short survey over the course of a year. You can also self enroll here.
The MTQIP surveys are designed to measure recovery that occurs after the patient leaves the hospital. Self-enroll now.
Surveys will be offered over the course of one year with the option to answer via phone, email, or text message.

Economic burden of traumatic injuries
Impact of caregivers (family and friends)
The Michigan Trauma Quality Improvement Program empowers participating hospitals and providers to improve care for patients with traumatic injuries such as falls, car/motorcycle accidents, or gunshot wounds.
MTQIP offers PROs to measure a patient's experience after leaving the hospital to help healthcare providers improve the quality of care.
Eligible patients will be contacted by the MTQIP Coordinating Center and can provide feedback on their healing process via phone, text, or email at regular intervals.
Patient-provided data will be shared with the patient’s hospital and treatment team; aggregate data will be shared with MTQIP participating hospitals. Any data that could reveal your private information will not be given to third-parties.
Just like your healthcare records, your information will be kept private and secure. Your personal information will only be accessed by members of the MTQIP team at our coordinating center.
